Job summary

Blue Skies of Texas Senior Living Community in San Antonio, TX is seeking a Hospice RN Weekend On Call Case Manager. The role provides skilled nursing care to hospice patients, oversees care delivery, and coordinates with the interdisciplinary team to ensure high-quality outcomes.

You will supervise LVNs and aides, educate families, participate in QA activities, and maintain HIPAA-compliant confidentiality while reporting to the Patient Care Manager.

Qualifications

  • Must be a Registered Nurse licensed in TX or compact state, with CPR certification and a valid TX driver’s license.
  • Excellent service skills, able to speak/read/write English, and able to work independently or in teams.
  • Preferred: Hospice certification.

Responsibilities

  • Provide skilled nursing care to hospice patients and families per plan of care.
  • Oversee quality of care delivery across disciplines and ensure safe, effective care.
  • Direct, assess, plan, implement and supervise patient care using nursing process and critical thinking.
  • Educate patient/family on disease, self-care, end-of-life care, and ethical concerns.
  • Provide field supervision to LVNs, HHAs, and attendants; participate in QA/PI activities.
  • Maintain confidentiality per HIPAA and participate in ongoing staff development.

As a Hospice RN Weekend On Call Case Manager, this position reports directly to the Patient Care Manager and is responsible for providing skilled professional nursing care to the patient and family. The incumbent oversees quality of care delivery by all disciplines and programs, assures delivery of optimal care safely and effectively, and identifies both existing and potential problems that impact the quality of patient care. Also ensures that provision of the highest quality of care to residents receiving Hospice services. This role is critical in directing, assessing, planning, implementing, and overseeing the care provided to patients. The incumbent occupies a position of confidence and trust and must maintain confidentiality in accordance with Health Insurance Portability and Accountability Act regulations.

What you will do in this role:
  • Provides patient care in accordance with accepted standards of nursing practice, agency policies and procedures, and the written plan of care to include initial and ongoing assessments of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial, spiritual and environmental needs, and on activities of daily living.
  • Receives patient referrals and applies specific criteria for admission and re-certification for hospice care to establish appropriate levels of care and the patient’s eligibility.
  • Coordinates, assesses, implements, and revises the individualized plan of care, to include: Managing discomfort and providing symptom relief, providing specialized nursing skills related to palliative and end-of-life care, creating plans of care and clinical solutions to patient problems utilizing the nursing process and critical thinking skills, and determining scope and frequency of services needed based on acuity and patient/family needs.
  • Consults with and educates the patient/family regarding the disease process, self-care techniques, end-of-life care, and the process for dealing with issues of ethical concern.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Participates in the coordination of all patient family services and prioritization of needs with the members of the interdisciplinary team.
  • Provides field supervision to Licensed Vocational Nurses, Home Health Aides, and personal attendants assigned to the incumbent’s case load according to regulations and guidelines.
  • Attends and participates in ongoing quality assurance and performance improvement activities as assigned.
  • Participates in staff development through attendance at in-service and continuing education programs.
  • Makes proficiency to include keeping current with trends and advancements of hospice care.
  • Provides specialized hospice training to other staff, family members, and informal caregivers to ensure proper level of patient care.
  • Performs other duties as assigned by Supervisor.
Qualifications
  • Required: Must be a Registered Nurse licensed by, and in good standing with, the state of Texas or an associated compact state allowing practice in Texas. Possess current Cardiopulmonary Resuscitation certification, as well as a valid Texas driver’s license with proof of liability insurance.
  • Required: Must possess excellent service skills and be able to speak, read, write, and follow verbal and written instructions in English. Incumbent must be a team player who is comfortable working independently and within groups. Awareness of special needs of the older adult is essential, as well as knowledge of physiological and psycho-social assessment with strong-problem solving skills and experience with hospice. Must possess the ability to organize, coordinate, and direct staff members in patient care, ensure attention to detail, initiative and judgment in determining needs and nursing service programs for patients, and display tact in dealing with personnel, medical and administrative staff, and the public.
  • Preferred: Certification in Hospice.
