Job summary

Care2You is seeking an experienced Hospice Weekend On-Call RN to provide compassionate, skilled nursing care to hospice patients at home during weekend hours. You will respond to patient and caregiver calls, perform triage, and make in-person visits as needed within the service area.

The role requires an Illinois RN license, CPR/BLS certification, strong clinical assessment and communication skills, the ability to work independently, and reliable transportation for Friday 5:00 PM – Monday 8:30

Qualifications

• Current Illinois RN license.
• Hospice or home health experience preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently and respond to urgent patient needs.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Current CPR/BLS certification.
• Ability to travel throughout the assigned service area.

Responsibilities

• Provide hospice nursing support during the weekend on-call period.
• Respond to patient and caregiver calls and assess changes in condition.
• Make in-person visits as needed for symptom management and patient needs.
• Provide end-of-life care and support to patients and families.
• Complete hospice admissions, visits, and death visits as needed.
• Collaborate with physicians and the interdisciplinary team.
• Address medication, equipment, and supply needs.
• Complete timely and accurate documentation in the EMR.
• Communicate important patient updates to the weekday hospice team.
