Key Responsibilities
Optimal Care Triage Registered Nurses are Registered Nurses or Licensed Practical Nurses who manage the coordination, supervision, and implementation of professional and supportive services to clients in a safe, cost-effective manner. The Triage Registered Nurse is responsible for the coordination of care during the episode of health care treatments.
In this role you will be responsible for:
- Communicates significant findings, problems, and changes in condition or environment to the physician and other personnel involved with patient care
- Initiates a plan for patient safety, using the patient, family, and community resources.
- Verifies the plan of care for the episode according to physician's orders, assessment data, and established standards and guidelines.
- Assumes primary responsibility for after hours supervisor including assessing, planning, coordinating, implementing, and evaluating the plan of care for patients and families.
- Manages and directs field team and weekend office staff, to ensure the delivery of safe, effective, and appropriate hospice services
- Coordinates services to ensure the availability and competency of staff to respond to organization referrals and provide assessment visits within 24 hours of referral
Required Qualifications
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse
- 2-3 years experience as a registered nurse in an appropriate clinical care setting
- 1 year of supervisory experience
- Nursing skills as defined by accepted nursing standards
- Requires interpersonal skills and ability to manage patient care episodes
- Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
- Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles
- Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines.
- Current and valid driver’s license
- Reliable transportation and valid automobile insurance coverage
- Sit, stand, bend, lift, and move intermittently
- Ability to lift at least 35lbs and bear the weight of an average adult effectively
Desired Qualifications
- BSN preferred
- Experience as a nurse in a hospice or home health environment preferred
- Familiarity with Home Care Home Base (HCHB)

- This position is per diem
- 5:00 pm to 8:00 am, Monday to Sunday
