Key Responsibilities:

  • Answer all incoming calls and communications and provide telehealth assessments, pain management, and symptoms.
  • Assess the patient and family's need for a home visit and scheduling through HCHB and notify the patient and family of the expected visit time.
  • Managing HCHB workflow including documentation of coordination notes and back office workflow.
  • Serves as an after-hours point of contact to ensure referrals are documented and ensure timely admissions.
  • Coordinate patient care and communication with interdisciplinary teams.

Key Qualifications:

Practical Nurse or Vocational Nurse licensed in the state of employment

Certified Hospice and Palliative Nurse and/or ASN/BSN preferred

Minimum 3 years of hospice nursing experience

Remote access Wi-Fi and phone

Demonstrated expertise in pain management, symptom control, crisis intervention, and emotional and pre-bereavement support

Excellent critical thinking skills and clinical judgement

Previous experience of operating and documenting in Homecare Homebase

Excellent verbal and written communication skills

Self-directed and able to work with minimal direct supervision

Ability to work outside of posted regular business hours, including overnights and weekends
