Residential Home Health and Hospice is seeking a Hospice Transitional Nurse Liaison in Troy, MI. This role partners with field staff to assess patients, coordinate care, and educate families about hospice services.
You will work with interdisciplinary teams to ensure smooth transitions and optimal patient outcomes. The ideal candidate holds an Associate degree in Nursing with at least two years of RN experience in acute care/oncology, and is proficient with Microsoft Office, EPIC, and HCHB.
