Job Description
Provides individual and/or group work services to patients and/or their families in various health care settings. Utilizes psychosocial assessment skills to deliver social work services.
Job Responsibilities
- Provides culturally competent direct service to patients and their families using various social work modalities to help individuals maximize their functioning.
- Uses high-risk criteria and consultation referrals to identify patients needing or benefiting from social work assessment and intervention. Advocates for individuals to ensure access to healthcare, assesses barriers, and offers solutions.
- Provides interdisciplinary team with assessment findings and suggests potential impacts on patient care.
- Delivers psychoeducation, counseling, and crisis intervention to patients and families.
- Develops and implements safe, timely treatment and discharge plans aligned with site expectations and patient preferences. Communicates with post-acute providers and payers as needed.
- Incorporates knowledge of community resources and entitlement programs into planning.
- Maintains proper professional patient care documentation according to site policies and procedures.
- Represents the department and profession on hospital/site committees and work groups.
- May supervise undergraduate and graduate social work students by assigning cases, monitoring progress through weekly meetings, and evaluating performance.
- Consults with program management regarding student training, progress, and development; meets with faculty advisors periodically.

Job Qualifications
- Master of Social Work degree required.
- Current license to practice as a Master Social Worker or Clinical Social Worker in New York State required, along with any specialized certifications needed.
- 1-3 years of relevant experience required.
