Job summary

West Texas Rehabilitation Center (WTRC) is seeking a PRN weekend Social Worker to deliver comprehensive social services to hospice patients and families. The role involves planning, directing, and coordinating a social work program in alignment with WTRC policies and standard practices.

You will collaborate with the IDT, assess needs, provide direct support, and maintain timely documentation while upholding confidentiality and professional standards.

Qualifications

• Master’s Degree in Social Work from a CSWE-accredited program or a BA/BS in Psychology, Sociology, or a related field with one year of healthcare experience and current state licensure.

Responsibilities

• Performs social services work with patients and families.
• Direct services for social, environmental, and emotional problems for patients and their families.
• Coordinate psycho-social needs with the Interdisciplinary Team (IDT).
• Establish service provider linkages with allied health and social service agencies.
• Maintain written records per hospice policies and confidentiality.
