Job summary

Concho Hearts Hospice is seeking a compassionate PRN Hospice Social Worker to support Greenville, TX and surrounding communities. You will conduct psychosocial assessments, provide counseling, and guide families through advance care planning while collaborating with an interdisciplinary team.

The role requires a social work degree and Texas licensure, with flexible scheduling and the opportunity for additional hours.

Qualifications

• Bachelor's or Master's degree in Social Work from an accredited program.

Responsibilities

• Complete psychosocial assessments for hospice patients and families.
• Identify emotional, social, financial, and practical needs affecting patient care.
• Provide emotional support, counseling, and guidance to patients and caregivers.
• Assist families with community resources, advance care planning, and end-of-life concerns.
• Support patients and families experiencing anticipatory grief, loss, and difficult transitions.
• Collaborate with nurses, chaplains, physicians, and other members of the interdisciplinary team.
• Participate in interdisciplinary team (IDT) meetings as needed.
• Advocate for patient needs, preferences, and dignity.
• Assist with crisis intervention and family support when appropriate.
• Complete accurate and timely documentation in the EMR system.

We're looking for a compassionate and dependable PRN Hospice Social Worker to join our team serving Greenville, TX, and surrounding communities.

What You'll Do:

What We're Looking For:

• Bachelor's or Master's degree in Social Work from an accredited program.
• Current Texas Social Work License (LBSW, LMSW, or LCSW).
• Previous hospice, home health, healthcare, or social work experience preferred.
• Hospice Social Work experience is a plus!
• Strong communication, assessment, and interpersonal skills.
• Compassionate and comfortable supporting patients and families during difficult situations.
• Ability to work independently while collaborating with an interdisciplinary team.
• Strong organizational and documentation skills
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel throughout Greenville and surrounding communities.
