Details

The Social Worker assists patients and families in coping with problems resulting from severe or long-term illness. They assess, diagnose, and treat patients' mental and social conditions, counsel individuals and/or families, and update case records.

Qualifications

• Assess psychosocial status and social/emotional factors related to the client’s illness, care needs, and treatment response.
• Provide direct social casework services, including goal setting, counseling, and intervention planning.
• Advise clients, families, and agency personnel about community resources, eligibility, and regulations.
• Participate in developing and updating the client’s care plan.
• Observe and report client responses to treatment and condition changes to the physician.
• Maintain communication with care team members regarding client progress.
• Keep accurate and timely client care records.
• Attend relevant care conferences, staff meetings, and agency committees.
• Demonstrate thorough knowledge of social work principles and methods.
• Understand individual and group behavior; work effectively with diverse populations experiencing social, economic, emotional, or health issues.
• Exercise independent professional judgment in complex situations.
• Establish and maintain effective relationships with clients, families, staff, and external agencies.
• Communicate effectively both orally and in writing.
• Master’s degree in Social Work required.
