Qualifications

• Master's degree in social work required.

• Agency affiliated counselor licensure also acceptable in WA.

Responsibilities

• Assess psychosocial needs, develop care plans, and coordinate family conferences.
• Plan and coordinate interventions; provide counseling and safe discharge planning; communicate with care team.
