Job summary

Alpha Omega Hospice is seeking a Licensed Master Social Worker (LMSW) to deliver patient-centered social work services in a per diem capacity in Carrollton, TX. You will assess psychosocial needs and support patients and families during hospice care, collaborating with an RN Case Manager and the interdisciplinary team.

Qualifications

• Master of Social Work from an accredited school.
• Licensed to practice as a Social Worker in Texas.
• Minimum 1 year hospice social work or healthcare setting experience.

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement personalized social work plans and collaborate with your Bereavement Coordinator.
• Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
• Provide empathetic counseling to patients and families.
