Caris HealthCare in South Carolina is seeking a Social Worker - LMSW to develop and deliver social work services, helping patients access funding and promoting interdisciplinary collaboration for emotional and social well-being.

The role requires LMSW licensure in SC and at least 1 year of healthcare social work experience, with opportunities in support of patients and families, and potential after-hours visits.

Qualifications

• Must hold LMSW licensure in South Carolina.
• Minimum of 1 year social work experience in healthcare.

Responsibilities

• Performs patient and family assessment within five days of admission.
• Collaborates with the IDT to formulate the Plan of Care.
• Documents visits and insurance coverage assistance for patients.
• Participates in IDT meetings at least every 14 days.
• Facilitates discharge/transfer when hospice care is no longer requested or appropriate.
• Explains Advanced Directives to Patients and Families and documents decisions.
• Willing to make after-hours visits when needed for admissions, death, or funeral.
• Identifies psychosocial problems and sets measurable goals.
