Job summary

Ennoble Care seeks an experienced, full-time Hospice Social Worker for the Sussex County, NJ region. The role focuses on psychosocial assessments, care planning with the IDG, and ongoing support for patients and families in a home-based hospice setting.

Requirements include an NJ-licensed LCSW or LSW, at least one year post-master’s experience in healthcare, and willingness to drive. Benefits include medical/dental/vision, PTO, holidays, and a 401(k) match.

Qualifications

• Licensed by NJ State Board of Social Work Examiners.
• Must be licensed as LCSW or LSW.

Responsibilities

• Conduct psychosocial assessment within 5 days of admission.
• Participate in IDG meetings and care plan development.
• Record and report changes in patient’s emotional and social factors.
• Maintain and submit clinical records (evaluation, care plan, notes).
• Evaluate patient and family response to social work intervention.
• Coordinate weekly visits with Case Managers for care coordination.
• Participate in IDG and QAPI meetings.
