You will conduct assessments, develop care plans with the interdisciplinary team, offer counseling, assist with advance directives and resources, and maintain compliant documentation.

Qualifications

• MSW from an accredited program.
• LCSW licensure in Colorado preferred.
• Minimum 1 year of social work experience in healthcare, hospice preferred.
• Strong interpersonal and teamwork abilities.
• Valid driver's license and reliable transportation.

Responsabilités

• Conduct initial and ongoing psychosocial assessments of patients and families.
• Develop individualized care plans with the interdisciplinary team.
• Provide supportive counseling and crisis intervention.
• Assist with advance directives, funeral planning, and community resources.
• Participate in interdisciplinary team meetings and care planning.
• Maintain accurate documentation per regulatory requirements.
• Ensure compliance with Medicare, Medicaid, and agency standards.
