Job summary

Emmanuel Hospice in Grand Rapids, MI, is seeking a full-time Social Worker to join our compassionate care team. You will provide holistic support to patients at the end of life and their families in the Grand Rapids area, working as part of an interdisciplinary team.

The ideal candidate holds an LMSW or LLMSW, demonstrates empathy, and can balance clinical duties with on-call responsibilities including nights, weekends, and holidays.

Qualifications

• Current LMSW or LLMSW degree required.
• Ability to work with an Interdisciplinary Team in the field.
• On-call responsibilities including nights, weekends, and holidays.
• Mileage reimbursement at the federal rate.

Responsibilities

• Providing holistic care to the dying and their loved ones in a service area around Grand Rapids, MI.
• Assessing the physical, psychological, social and spiritual needs of terminally-ill patients.
• Assisting the patient and family with the Plan of Care to meet their needs and monitor goals, while suggesting appropriate interventions.
• Working with an Interdisciplinary Team to plan, implement, and evaluate medical care.
