Job summary

ViaQuest Hospice is seeking a licensed Master’s level Social Worker to join our care team. The role emphasizes comprehensive patient assessments, scheduled patient visits, crisis intervention and counseling, and advocacy for patients and families. Experience in medical social work and hospice is preferred, with travel across the service area expected.

Qualifications

• Master's degree in Social Work is required.
• License in Social Work from the State of Ohio is preferred.
• Experience in Medical Social Work is preferred.
• Experience within the Hospice field is preferred.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.

Responsibilities

• Performing comprehensive patient assessments.
• Providing patient visits as scheduled.
• Providing short term crisis intervention and individual or family counseling as needed.
• Utilizing our electronic medical record system, Brightree, to complete documentation.
• Serving as a patient and family unit advocate.
