Qualifications

• Bachelor’s Degree in Social Work or related major, or equivalent education/experience.
• Competency in working with culturally diverse populations.
• At least one year in a health care setting, preferably hospice.
• Valid LMSW or LCSW license.
• Driver’s license, car insurance, and registration.

Responsibilities

• Assess psychosocial status of patients and families/caregivers and communicate findings to the care team.
• Conduct social evaluations, plan interventions, and provide crisis counseling as needed.
• Educate teams on cultural needs and family dynamics related to health care.
• Maintain clinical records for referred patients and provide information/referrals to services.
• Serve as liaison between patients, families, and community agencies; assist with discharge planning.
