Job summary

Qualifications

• Masters Social Work required.
• 2+ years in social work related field, preferably healthcare or community-based services.
• Licensed MSW (LMSW) upon hire.
• CPR/AED/BLS training within 14 days of start.

Responsibilities

• Assess and intervene to meet psychosocial needs of hospice and home health clients and families.
• Document assessments, interventions, and contacts.
• Support bereavement counseling and group facilitation as needed.
• Participate in IDT meetings and coordinate with clinical staff.
• Utilize community resources to address patient and family needs.
• Demonstrate professional growth through ongoing education and training.
