Qualifications

• MSW or BA in social work from CSWE-accredited program
• Meets personnel qualifications referenced in 418.114(b)(3)
• Minimum 1 year of healthcare experience; hospice experience preferred
• Understand hospice philosophy and issues around death/dying
• CPR certification required
• Must have a valid driver's license with insured vehicle

Responsibilities

• Assess psychosocial status of patients and families; communicate findings to care team
• Conduct social evaluations including family dynamics and caregiver abilities; provide crisis intervention
• Educate interdisciplinary groups on cultural needs and family roles
• Maintain clinical records for referred patients
• Assist in preparation of advanced directives and referrals
• Provide information to patients, families, and community agencies
• Serve as liaison between patients, families, and community agencies
• Develop and maintain contracts with agencies as resources
• Participate in care planning with patient and family; attend interdisciplinary meetings
• Support physicians and team in psychosocial factors related to health and end-of-life issues
• Participate in quality improvement activities
