Job summary

UnitedHealth Group seeks a Social Worker to provide medical social services in home hospice visits under physician guidance. You will support patients and families, help develop care plans with the interdisciplinary team, and connect them with community resources.

Requirements include a Master's in Social Work from a CSWE-accredited program, an Ohio Social Worker license, and a valid driver's license with access to a vehicle.

Qualifications

  • Master's Degree from a school of Social Work accredited by the Council on Social Work Education (CSWE).
  • Current Ohio Social Worker license required.
  • Driver's license and access to a dependable vehicle or transportation.

Responsibilities

  • Assist patients and families to understand social and emotional factors impacting health and coping.
  • Contribute to an integrated Plan of Care with the health care team.
  • Guide patients to community resources and provide ongoing support within a home hospice setting.

As the Social Worker, provides medical social services under the direction of a physician and Interdisciplinary Group to assist in the understanding of significant social and emotional factors related to the patient's health status and in development of coping mechanisms.

  • Full-time position providing in-home visits for hospice patients.
  • RequiredQualifications: current Ohio Social Worker License & Master's Degree from a school of Social Work accredited by the Council of Social Work
  • Standard business hours are Monday-Friday, 8:0 AM-4:30 PM.
  • Travel required within a 50-mile service radius to provide patient care
  • Hospice experience preferred
Primary Responsibilities:
  • Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
  • Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
  • Instructs health care team members on community resources available to assist patients on a as needed basis

Required Qualifications:
  • Master's Degree from a school of Social Work accredited by the Council of Social Work
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
Additional State Specific Requirements:
  • OH : Licensed as a Social Worker from the State Board of Social Work.
Preferred Qualifications:
  • 1+ years of social work experience in a health care setting
  • Current CPR certification or ability to complete within 90 days of hire
  • Licensed Social Worker in the state of residence

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $66,400 to $99,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
