Job summary

Caretenders Hospice, a part of LHC Group, under UnitedHealth Group, is seeking a Hospice Social Worker for in-home visits in the Dayton area. You will support patients and families, and help develop and implement a care plan with the Interdisciplinary Group.

Required is a Master’s in Social Work and OH Licensed Social Worker; healthcare experience is preferred. Travel within a 50-mile radius is expected. Salary ranges from $66,400 to $99,600 with full benefits.

Qualifications

  • Master's Degree from a school of Social Work accredited by the CSWE.
  • Current Ohio Social Worker license or eligible to obtain.
  • 1+ years of social work experience in a healthcare setting.
  • CPR certification or ability to obtain within 90 days of hire.
  • Licensed Social Worker in the state of residence.

Responsibilities

  • Assist patients and families to understand social and emotional factors affecting health.
  • Develop a comprehensive Plan of Care with the healthcare team.
  • Coordinate community resources for patient care as needed.

As the Social Worker, provides medical social services under the direction of a physician and Interdisciplinary Group to assist in the understanding of significant social and emotional factors related to the patient's health status and in development of coping mechanisms.

  • Full-time position providing in-home visits for hospice patients.
  • RequiredQualifications: current Ohio Social Worker License & Master's Degree from a school of Social Work accredited by the Council of Social Work
  • Standard business hours are Monday-Friday, 8:00 AM-4:30 PM.
  • Travel required within a 50-mile service radius to provide patient care
  • Hospice experience preferred
Primary Responsibilities:
  • Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
  • Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
  • Instructs health care team members on community resources available to assist patients on a as needed basis

Required Qualifications:
  • Master's Degree from a school of Social Work accredited by the Council of Social Work

Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Additional State Specific Requirements:

OH : Licensed as a Social Worker from the State Board of Social Work.

Preferred Qualifications:
  • 1+ years of social work experience in a health care setting
  • Current CPR certification or ability to complete within 90 days of hire
  • Licensed Social Worker in the state of residence

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. The salary for this role will range from $66,400 to $99,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
