VITAS Healthcare is seeking a compassionate hospice social worker to support patients and their loved ones at a vulnerable time. You will help ensure end-of-life plans are organized, honoring patients' wishes for finances, funeral arrangements, and more.

Responsibilities include psychosocial assessments, individual and family counseling, bereavement support, and coordination with the interdisciplinary team. Travel to patients' homes is common; regular office visits occur with the team.

Qualifications

• Masters of Social Work from CSWE-accredited program.
• At least one year of social work experience in a healthcare setting.
• Valid licensure and/or certification as required by your state.
• Reliable transportation.

Responsabilités

• Provide emotional and logistical support to patients and families during end-of-life.
• Perform psychosocial assessment and develop care plans with clinical teammates.
• Provide individual and family supportive counselling.
• Deliver psychotherapy if credentialed.
• Deliver bereavement care to families after patient’s passing.
• Refer patients and families to community resources as needed.
• Maintain accurate and timely documentation.
