Job summary

Brighton Hospice in Atlanta seeks a dedicated Hospice Social Worker to provide compassionate psychosocial support to patients, families, and the care team. You will advocate for patient needs, connect families with community resources, and participate in IDT meetings to ensure holistic care.

The role requires a Social Work degree compliant with CHAP/state regulations, with a focus on navigating illness, loss, and end-of-life transitions.

Qualifications

• Social Work degree compliant with CHAP and state regulations.
• Assess psychosocial, emotional, spiritual, and socioeconomic needs and incorporate into plan of care.
• Counsel patients and families with emotional support and crisis guidance.

Responsibilities

• Provide compassionate psychosocial support to patients, families, and team members.
• Advocate for patients and families to identify barriers to care and develop solutions.
• Educate patients and families about available community resources and connect them with services.
• Participate in interdisciplinary team (IDT) meetings and collaborate with care team.
• Maintain accurate documentation in accordance with hospice standards and regulatory requirements.
