Job summary

Brighton Hospice in Brookhaven, GA is seeking a compassionate Hospice Social Worker to join our CHAP-certified team. You will assess patients and families, provide emotional support, educate about community resources, and advocate to remove barriers to care as part of the interdisciplinary team.

You will collaborate with physicians, nurses, and other care providers to ensure a holistic, patient-centered plan of care and thorough documentation in compliance with hospice standards.

Qualifications

• Assess patients’ and families’ psychosocial, emotional, spiritual, and socioeconomic needs and incorporate findings into the individualized plan of care.
• Counsel patients and families while providing emotional support, crisis intervention, and guidance as they navigate illness, loss, grief, and significant life transitions.
• Educate patients and families about available community resources and connect them with appropriate programs, services, and support.
• Advocate for patients and families to identify barriers to care and the hospice team to develop appropriate solutions to ensure their needs, preferences, values, and goals are heard and addressed.
• Participate in interdisciplinary team (IDT) meetings and collaborate with physicians, nurses, chaplains, aides, facility staff, and other members of the care team.
• Maintain accurate, thorough, and timely documentation in accordance with hospice standards and regulatory requirements.
