Adoration Health in Pittsburgh, PA is seeking a compassionate Social Worker to join our hospice-focused team. You will assess psychosocial needs, provide counseling, coordinate community resources, and help families navigate end-of-life planning with dignity and respect.

Qualifications

• Master’s in social work (MSW) or bachelor’s in social work (BSW) accredited by CSWE.
• Minimum of 1 year experience in health care; hospice experience preferred.
• Current licensure to practice in state of operation.
• Good verbal and written communication and organizational skills.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety; assist with alternative care plans.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Provide referrals and practical support for patients and families.
• Serve as liaison with community resources and agencies.
• Collaborate with team members to support holistic patient care.
• Support funeral, financial, and legal planning with families.
