Job summary

Valir Hospice in Oklahoma City, OK is seeking an experienced Social Worker (MSW) to join the interdisciplinary team. The role includes patient and family assessment, care planning, and coordination of psychosocial support within policy guidelines.

Typical duties involve admissions evaluation, documentation, after-hours on-call coverage, and participation in community outreach. 1–2 years hospice experience is preferred; LCSW is a plus.

Qualifications

• Master of Social Work (MSW) is required.
• 1–2 years of hospice or long-term care experience preferred.
• LCSW licensure is preferred.
• Excellent verbal and written communication skills.
• Strong organizational and time-management abilities.

Responsibilities

• Participates in referral evaluation to ensure appropriate admissions.
• Assesses patients and families for psychological needs and concerns.
• Contributes to the interdisciplinary team plan of care.
• Coordinates psychological care to meet patient/family needs.
• Documents visits and phone calls in clinical records.
• Attends team meetings and participates in hospice programs for staff and the community.
• Responds to patient/family needs after hours when on call.
