Good Samaritan Hospice in Lynchburg, VA seeks a Medical Social Worker to assess, counsel, and plan care for patients and families. You will collaborate with the interdisciplinary team to develop, implement, and evaluate care plans, coordinate resources, and support advance care planning and advocacy.

Qualifikationen

• Master’s Degree in Social Work is required.
• Two years of social work or counseling experience, including healthcare setting.
• LCSW or hospice/palliative care certification preferred.
• Experience with psychosocial assessments, counseling, case management, and crisis intervention.

Aufgaben

• Conduct psychosocial assessments and develop plans of care.
• Provide counseling and emotional support to patients, caregivers, and families.
• Collaborate with interdisciplinary team to develop, implement, and evaluate care plans.
• Coordinate community resources and assist with psychosocial needs.
• Assist with advance care planning and end-of-life wishes.
• Advocate for patients while collaborating with physicians, hospitals, long-term care facilities, and community agencies.
• Participate in family meetings and care conferences.
• Complete timely documentation in line with Medicare regulations and agency standards.
• Support hospice admissions, discharges, and transitions of care.
• Engage in quality improvement and continuing education.
