Job summary

Moments Hospice in Wisconsin is seeking a licensed social worker (LSW) to provide compassionate psychosocial care for terminally ill patients and their families. You will conduct assessments, coordinate services, and collaborate with an interdisciplinary team to support patient comfort and family needs.

Qualifications

• Master’s degree in Social Work preferred or Bachelor’s with healthcare experience.
• Current Social Worker license (required).
• Two years of social work experience in healthcare or social services setting.
• Knowledge of community resources and hospice philosophy.
• Strong organizational and communication skills.
• Valid driver’s license and auto insurance.
• Hospice experience is a plus.

Responsibilities

• Provide psychosocial services to terminally ill patients and families.
• Contact patients, families, and caregivers within 48 hours of admission.
• Perform case manager duties and psychosocial assessments within 24 hours of completion.
• Assess and address patient and family psychosocial needs regularly.
• Participate in weekly interdisciplinary team meetings.
• Assist with funeral arrangements if requested.
• Provide counseling during terminal illness and bereavement periods.
