About Our Hospice and Palliative Care Programs

We provide services to help patients and families manage life-limiting illnesses and end-of-life care, either in the community or in facilities.

Responsibilities

• Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, or nursing home settings, including pre- and post-bereavement phases.
• Serve as a social work member of the hospice interdisciplinary team and as a liaison between hospice staff and inpatient/nursing home staff.
• Develop social work plans of care, set goals, and make recommendations based on patient and family needs for patients admitted directly to inpatient units or nursing homes.
• Create discharge plans and social work strategies for patients in inpatient facilities.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of one year experience in healthcare social work.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS Driver's license.
• Ability to work within an interdisciplinary team.
• Excellent organizational, communication, and listening skills.
• Proficiency in inputting clinical data.
• Bilingual in English and Chinese (Mandarin or Cantonese) required.
