Athena Hospice of Rhode Island is seeking a per diem Hospice Social Worker to help plan, organize, and direct social services for residents statewide.

You will assess psychosocial status, educate the team on cultural needs, participate in interdisciplinary care planning, and support patients and families through hospice care and end-of-life issues. Licensure and experience are preferred; valid driver's license required; English proficiency; up to 20 hours per month.

Qualificações

• Must hold a state social work license.
• Minimum of 1 year of clinical healthcare or hospice experience preferred.
• Knowledge of hospice philosophy and death/dying issues preferred.
• Must have valid driver's license and insured vehicle.
• Ability to read, write, speak, and understand English.

Responsabilidades

• Assess the psychosocial status of patients and families/caregivers
• Provide an assessment of the patient's identified residence
• Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, and plans intervention based on evaluation findings
• Counsel patients and family/caregivers as needed in relation to stress and other identified coping difficulties
• Educate the team on any special needs related to the culture of the patient and family
• Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings
• Assist the physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues

Hospice Social Worker

We service the entire state of Rhode Island. Up to $45 an hour, depending on license and experience, with weekly pay. Looking for someone to help up to 20 hours a month.

As a per diem licensed social worker, you will play a vital role in planning, organizing, and directing all required social services to meet the needs of our residents. Your expertise will ensure that the medically related emotional and social needs of each individual are met and maintained on an individual basis.

Responsibilities:

• Assess the psychosocial status of patients and families/caregivers
• Provide an assessment of the patient's identified residence

Requirements:

• Must be a licensed driver with an insured automobile
• Must be able to read, write, speak, and understand the English language
• Must possess the ability to make independent decisions when circumstances warrant such action
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and general public
