Job summary

Athena Hospice of Rhode Island is seeking a per diem Hospice Social Worker to join our statewide team. You will assess psychosocial needs, plan and coordinate social services, and collaborate with physicians and care staff to support residents and families through illness and end‑of‑life care.

Requirements include a Bachelor's in social work and state licensure; clinical hospice experience is preferred. Valid driver’s license and English proficiency are required.

Qualifications

• Bachelor's in social work or related field required.
• State social work license required.
• One year of clinical healthcare and/or hospice experience preferred.
• Must be able to read, write, speak, and understand English.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers.
• Provide an assessment of the patient's identified residence.
• Carry out social evaluations, including family dynamics and caregiver abilities; intervene as needed.
• Counsel patients and families on stress and coping difficulties.
• Educate the team on cultural needs related to patient and family.
• Participate in development of the individualized plan of care with patient and family.
• Assist the physician and team in understanding social and emotional factors related to health and death/dying.

Hospice Social Worker

Job Type: Per diem

Company: Athena Hospice of Rhode Island

We service the entire state of Rhode Island. Up to $45 an hour, depending on license and experience, with weekly pay. Looking for someone to help up to 20 hours a month.

As a per diem licensed social worker, you will play a vital role in planning, organizing, and directing all required social services to meet the needs of our residents. Your expertise will ensure that the medically related emotional and social needs of each individual are met and maintained on an individual basis.

Responsibilities:

• Assess the psychosocial status of patients and families/caregivers
• Provide an assessment of the patient's identified residence
• Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, and plans intervention based on evaluation findings
• Counsel patients and family/caregivers as needed in relation to stress and other identified coping difficulties
• Educate the team on any special needs related to the culture of the patient and family
• Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings
• Assist the physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues

Requirements:

• Bachelor's in social work or related field required
• State social work license required
• One (1) year of clinical healthcare and/or hospice experience preferred
• An understanding of hospice philosophy and issues of death/dying preferred
• Must be a licensed driver with an insured automobile
• Must be able to read, write, speak, and understand the English language
• Must possess the ability to make independent decisions when circumstances warrant such action
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and general public
