Role Description

The Hospice Social Worker is a full-time, on-site role based in Phoenix, AZ, responsible for supporting patients and families throughout the hospice care experience. Responsibilities include conducting psychosocial assessments, helping develop care plans, and coordinating services that address emotional, social, and financial needs. The role involves providing grief counseling and emotional support, assisting with discharge planning and transitions of care as needed, and advocating for patient rights and preferences. The Hospice Social Worker collaborates closely with the interdisciplinary team, including nurses, physicians, chaplains, and other care providers, to ensure holistic, person-centered support. Additional tasks include maintaining accurate and timely documentation, connecting families with community resources, and participating in team meetings and quality improvement activities.

Qualifications
  • Demonstrated proficiency in Medical Social Work and Social Work within healthcare or hospice settings.
  • Experience with End-of-Life Care, including psychosocial support for patients and families facing serious illness.
  • Skills in Discharge Planning and care coordination to ensure safe and smooth transitions between care settings.
  • Training and experience in Grief Counseling and bereavement support for diverse individuals and families.
  • Master’s degree in Social Work from an accredited program; current state licensure is desirable but not required.
  • Strong communication, active listening, and conflict-resolution skills, with the ability to work effectively in an interdisciplinary team.
  • Cultural humility and the ability to provide trauma-informed, person-centered care to individuals from diverse backgrounds.
  • Reliable organization, documentation, and time-management skills, with the ability to manage a caseload in a fast-paced environment.
  • Previous hospice, palliative care, or medical social work experience preferred.
