An established industry player is seeking a compassionate Hospice Social Worker to join their dedicated team in Virginia. Hospice Social Worker - The Social Worker provides a wide variety of direct and indirect social work services to patients/caregivers/families in accordance with the individualized plan of care, as prescribed by the physician. The Hospice Social Worker actively participates in coordination of all aspects of the patient’s care, in accordance with current professional standards and practice. The Social Worker participates in ongoing interdisciplinary comprehensive assessment; develops and evaluates the Plan of Care; contributes to patient and family counseling and education and participates in the QUAPI program and hospice sponsored in-service training.
Schedule: full time, Monday-Friday
Location: Prince William and Fauquier Counties, VA

The Hospice Social Worker, an active member of the Capital Caring Health Interdisciplinary Team, provides a wide variety of direct and indirect social work services to patients/caregivers/families in accordance with the individualized plan of care, as prescribed by the physician. The Social Worker actively participates in coordination of all aspects of the patient’s care, in accordance with current professional standards and practice. She/he participates in ongoing interdisciplinary comprehensive assessment; develops and evaluates the Plan of Care; contributes to patient and family counseling and education, and participates in the QUAPI program and hospice sponsored in-service training.
Experience Requirements:
Two years of master’s level experience in an interdisciplinary health care setting preferred.
Education Requirements:
Must have atMaster's degree in Social Work from a school accredited by the Council on Social Work Education.
Licensing Requirements:
Must be licensed or license eligible in compliance with jurisdiction of practic (LCSW, LICSW).
Driver's License:
Must have driver's license in good standing and access to a vehicle during working hours.
