Summary:

The Hospice Social Worker assists patients and families with psychosocial, emotional, and resource needs related to end-of-life issues.

The work schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m., with on-call responsibilities.

Recent graduates are encouraged to apply.

The salary ranges from $28.35 to $35.35 for LMSW with a sign-on bonus. Recent graduates and LLMSWs may start at a lower rate. The role includes a monthly cell phone stipend and mileage reimbursement.

Our office is located in Holland, serving Allegan, Ottawa, parts of Kent and Muskegon counties, and the northern part of Van Buren County. The candidate must be willing to travel routinely within this area.

Responsibilities include:

• Completing admission and transfer processes, explaining hospice services and insurance benefits, and completing paperwork.
• Providing counseling and education on psychological, emotional, and social aspects of death and dying.
• Helping patients and families identify and access resources for environmental, financial, legal, or psychological needs.
• Documenting all contacts and activities accurately and timely.

Qualifications:

• Master's degree in social work from an accredited school and current Michigan clinical social work licensure.
• Valid Michigan driver's license and transportation.
• Sound clinical judgment and ability to work independently.
• Experience with end-of-life care preferred.
• Effective verbal and written communication skills, including concise charting.
