Job summary

Moments Hospice is seeking a Hospice Social Worker to support patients and families in home and facility settings, providing psychosocial support, assessments, and care planning to address needs and bereavement. You will connect families with community resources and participate in team meetings, ensuring compassionate care throughout the hospice journey.

The role requires an MSW (preferred) or BA in Social Work, 1+ year healthcare social work, a current SW license, and a valid driver’s license;

Qualifications

• Master's degree in Social Work preferred; Bachelor's degree in Social Work required
• 1+ year of social work experience in a healthcare setting or social services agency
• Current Social Worker license (required)
• Hospice experience a plus
• Knowledge of community resources, including state and federal financial programs and eligibility requirements
• Understanding of hospice philosophy and the principles of death and dying
• Strong organizational and communication skills
• Valid driver's license and auto insurance (required)

Responsibilities

• Provide psychosocial support and counseling to patients and families throughout the hospice journey
• Complete psychosocial assessments and document them in the patient's chart
• Regularly assess and reassess the psychosocial, anticipatory grief, and pre-bereavement needs of patients and families
• Carry out the Plan of Care, including social service goals, supportive counseling, problem solving, community referrals, and bereavement care
• Connect families with community resources, including financial programs and eligibility support
• Take part in weekly interdisciplinary team meetings
• Help with funeral arrangements and provide counseling at the time of death when requested
