Heritage Hospice is recruiting a FULL-TIME Medical Social Worker to support our home care patients. The role requires a Master’s Degree in Social Work and at least one year of health care social work experience, with hospice experience preferred.

Responsibilities

  • Assist the physician and care team in understanding social and emotional factors affecting patient health and help develop the plan of care.
  • Assess psychosocial situation of patients and caregivers and support adherence to medical recommendations.
  • Provide information and referrals for practical and environmental needs, including financial assistance and community resources.
  • Assist patients and families with personal and environmental difficulties affecting care access.
  • Consult with family, physician, and other staff to promote coordinated, quality patient care.
  • Document evaluations, plans of care, notes and progress reports.

Heritage Hospice is recruiting a FULL-TIME Medical Social Worker to add to our team! The Medical Social Worker will assist our team on as needed basis helping support our home care patients.

Job Duties:
  • Assist the physician and other team members in understanding the significant social and emotional factors related to the patient’s health problems, perform evaluations, and participate in the development of the plan of care for the patient
  • Assess psychosocial situation of patient/caregivers and help them understand and follow the medical recommendations made by the care team
  • Provide information and referral services for patients and families regarding practical and environmental needs, including financial assistance programs and resources, and serve as the liaison between patients/families and community agencies
  • Assist the patient and family with personal and environmental difficulties which pre-dispose toward illness or interfere with obtaining maximum benefits from medical care
  • Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
  • Performs and submits required documentation of patient’s evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders

  • Candidate(s) must have a Master’s Degree from a Social Work accredited by the Council on Social Work Education
  • One (1) year of Social Work experience in a health care setting, hospice experience preferable.
  • Must be able to drive to patient’s current residency (assisted living facility, house, etc.) and have a valid driver\'s license and auto insurance
