Job summary

Family Home Health is seeking a compassionate Medical Social Worker to join our interdisciplinary team. You will assist patients and families to cope with psychosocial challenges and coordinate care to meet health goals.

The ideal candidate has at least 1 year of Medical Social Work experience and a valid LCSW, plus strong communication skills and comfort with electronic documentation. This role covers everything south of Addison, IL and is three days per week with flexible days.

Qualifications

• Minimum of 1 year of Medical Social Work experience and possess a LCSW.
• Present a compassionate and caring demeanor with the ability to build a rapport with clients.
• Possess excellent communication and customer service skills, be well organized, and be comfortable with electronic documentation.
• HCHB EMR experience a plus
• Prior Hospice care experience a plus

Responsibilities

We are looking for a compassionate and experienced Medical Social Worker to join our interdisciplinary team. As a Medical Social Worker, you will assist patients and families to cope with psychosocial challenges to achieving their health care goals. You will utilize a holistic approach to assess patient and family/caregivers needs relative to coordinating appropriate care. You will develop, implement, and update interdisciplinary treatment plans, consult with and educate patient and family and facilitate referrals to other healthcare professionals and programs. You will participate in team meetings and required in-service trainings.
