Job summary

Interim HealthCare of the Twin Cities is seeking a Hospice Social Worker to provide individualized and coordinated hospice care, ensuring safety and comfort for patients and families. The role offers independence to visit patients in their home environment, requiring a Master's degree in Social Work and active Minnesota licensure.

Qualifications

• Licensed to practice as a Social Worker in the state of Minnesota.
• Minimum of one year's experience in health care.
• Hospice experience preferred.
• Valid driver’s license and reliable transportation required.

• Provide individual or family counseling to reduce barriers to care.

Job description

Hospice Social Worker

Provide individualized and coordinated hospice care to bring safety and comfort to the patients and families. You will have independence and flexibility in the field to visit patients one on one in their home environment.

Hours

Full‑time, Days, Monday - Friday, Tentative On‑Call rotation dependent upon staffing levels.

Pay and Benefits

Territory

Our office is in Roseville, MN and the Social Worker will have a general territory within our service area where they will provide in‑home hospice care to the patients. The territory for this position would primarily be in the Southern section of the Twin Cities Metro area.

Required Qualifications

• Graduate of a Master’s program in Social Work accredited by the Council on Social Work Education.
• Licensed to practice as a Social Worker in the state of Minnesota. License is active and in good standing.
• Minimum of one (1) year's experience in health care.
• Hospice experience preferred.
• Able to travel within geographic area serviced by office. Valid driver’s license, reliable transportation, and proof of auto insurance are required.

Responsibilities

• Conduct an initial psycho‑social assessment of assigned patients/families within five (5) calendar days of admission to contribute to the comprehensive assessment of the patient’s/family’s needs and the Interdisciplinary Group (IDG) plan of care.
• Provide individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care.
• Identify special needs related to religious or cultural diversity and how these impact the implementation of the plan of care.
• Evaluate identified financial or environmental concerns and direct the patient/family to appropriate community resources, providing follow‑up and acting as a liaison when indicated. Provide crisis intervention support to the patient/family, members of a facility caregiver group or the IDG.
• Assist the patient/family in electing advance directives, securing durable power of attorney, making funeral arrangements and other financial, legal and healthcare aspects of end‑of‑life choices.
