Ovation Hospice of Denver is seeking a compassionate Part-Time Hospice Social Worker (MSW) to provide psychosocial assessments, counseling, and care coordination in the Denver metro area. This field-based role offers flexible scheduling and meaningful impact for patients and families facing end-of-life decisions.

You will work with an interdisciplinary team, coordinate resources, and support families through emotional and practical challenges.

Formación

• MSW degree from an accredited program.
• Current Colorado LSW or LCSW license (LCSW preferred).
• Strong assessment, crisis intervention, and communication skills.
• Ability to work independently in the field and manage a caseload.
• Valid Colorado driver's license and reliable transportation.
• Proficiency with electronic medical records.

Responsabilidades

• Complete initial and ongoing psychosocial assessments within required timeframes.
• Provide emotional support and short-term counseling to patients, families, and caregivers.
• Assist families with advance directives, MOST forms, funeral planning, and end-of-life decisions.
• Connect families with community resources (Medicaid, VA benefits, caregiver support, placement).
• Participate in IDG meetings and contribute to care plans.
• Identify caregiver stress and risk factors; escalate as needed.
• Coordinate with bereavement team for post-death support.
• Document visits accurately in the EMR per Medicare/CHAP standards.

The Role

We’re looking for a compassionate, skilled Medical Social Worker to join our team part-time. You’ll provide psychosocial assessment, counseling, and care coordination for patients and families, and help guide them through the emotional, practical, and financial challenges that come with a terminal diagnosis. This is a field-based role with a flexible schedule.

What You’ll Do

• Complete initial and ongoing psychosocial assessments within required timeframes
• Provide emotional support and short-term counseling to patients, families, and caregivers
• Help families with advance directives, MOST forms, funeral planning, and end-of-life decisions
• Connect families with community resources, including Medicaid, VA benefits, caregiver support, and placement options
• Take part in Interdisciplinary Group (IDG) meetings and contribute to individualized plans of care
• Identify caregiver stress, safety concerns, and risk factors, and **escalate** them appropriately
• Coordinate with our bereavement team to support families after a death
• Document visits accurately and on time in the EMR, in line with Medicare and CHAP standards

What We’re Looking For

• Master’s degree in Social Work (MSW)
• Current Colorado license (LSW or LCSW); LCSW preferred
• Hospice, palliative care, home health, or medical social work experience preferred
• Strong assessment, crisis intervention, and communication skills
• Comfort working independently in the field and managing a caseload
• Valid Colorado driver’s license, reliable transportation, and current auto insurance
• Proficiency with electronic medical records

Schedule & Compensation

• Pay: $30–$35per hour / per visit, depending on experience and licensure
• Mileage stipend: $180/ Month
