Your Role as a Social Worker at Gallagher Hospice:

  • Provide compassionate care to hospice patients and their families, offering psychosocial support during the end-of-life journey.
  • Conduct comprehensive psychosocial assessments and develop care plans that address emotional, family, and cultural factors affecting the patients' well-being.
  • Collaborate with an interdisciplinary team to ensure the delivery of comprehensive, holistic care that meets the patients' needs.
  • Offer counseling, crisis intervention, and support to patients and families facing the emotional and practical challenges of terminal illness.
  • Educate patients and families on advanced directives and assist in planning for end-of-life decisions.
  • Serve as a liaison between patients, families, and community agencies, helping to connect them with valuable resources and services.

Qualifications:

  • Masters degree in Social Work from an accredited program
  • Minimum of one (1) year of healthcare experience; hospice experience preferred
