Job summary

VitalCaring Group seeks a Hospice Speech-Language Pathologist to plan, organize, and deliver compassionate therapy focused on comfort, communication, and quality of life within home health and hospice settings.

Guided by our values, you will help patients and families navigate meaningful goals at the end of life, collaborating with physicians and an interdisciplinary team to ensure coordinated care and support for families in our care network.

Qualifications

• Master’s degree in Speech-Language Pathology from an approved program, including required internship.
• Meets education and experience requirements for the CCC from ASHA.
• Certified and licensed to practice Speech-Language Pathology in the state of practice.
• Current CPR certification.
• Licensed driver with an insured, reliable automobile meeting organizational requirements.
• Minimum of one year of clinical SLP experience.
• Understanding of hospice philosophy and comfort-focused care.

Responsibilities

• Conduct evaluations and assess the home environment to determine equipment and speech/language needs.
• Identify functional speech and communication deficits; establish an individualized plan of care to improve or maintain essential function.
• Administer speech therapy programs using specialized techniques and equipment.
• Recommend communication devices or assistive aides as needed.
• Instruct patients, families, caregivers, and agency personnel on treatment plans, communication strategies, and safe care techniques.
• Collaborate closely with referring physicians and the multidisciplinary team to ensure comprehensive, coordinated care.
