Job summary

Serenity Hospice Care is seeking an Assistant Team Lead RN to support the clinical leadership team and guide our hospice nursing staff in Essex/Union County, NJ. This role blends direct patient care with leadership responsibilities and mentoring of RN Case Managers.

The Assistant Team Lead will help coordinate daily clinical operations, ensure quality care, and maintain compliance with hospice regulations and company policies while serving as a clinical resource for the team.

Qualifications

• Current, unrestricted RN license in New Jersey.
• Associate Degree in Nursing required; BSN preferred.
• Minimum of 2–3 years of professional nursing experience.
• Previous hospice, home health, palliative care, or end-of-life care experience preferred.
• Leadership, charge nurse, or supervisory experience preferred.
• CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Strong clinical assessment and critical-thinking skills.
• Excellent written and verbal communication skills.
• Strong organizational and time-management abilities.
• Ability to work independently while collaborating with an interdisciplinary team.
• Proficiency with electronic medical records and standard computer applications.

Responsibilities

• Assist the Team Lead with daily clinical operations and coordination of the hospice nursing team.
• Provide guidance, support, and mentorship to RN Case Managers and other clinical staff.
• Assist with assigning and coordinating patient visits based on needs and coverage.
• Monitor patient care to promote continuity, quality, and compliance with the plan of care.
• Respond to urgent clinical situations and assist with escalation of patient-care concerns.
• Participate in interdisciplinary group meetings and clinical meetings as assigned.
• Review clinical documentation for completeness and accuracy.
• Ensure timely completion of assessments, care plans, orders, and recertifications.
