Admit, Recert, SCIC, Post-hospital and Discharge visits as assigned Supervisory hospice visits to patients Preparing home health aide care plans Check charts for completeness and accuracy Attend staff meetings On-call rotation Other assigned duties.**JOB QUALIFICATIONS**Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of NursingLicensure: Current Texas State license as a Registered Nurse, current Texas Driver’s LicenseExperience: One year experience as nurse in a clinical care setting.Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of State and Federal regulations. Good interpersonal skills.Transportation: Reliable transportation.
