Job summary

Qualifications

• BSN from an accredited institution.
• Valid RN license in California.
• Minimum of two years of clinical nursing experience, preferably in hospice or palliative care settings.
• Experience coordinating with interdisciplinary teams.

Responsibilities

• Conduct comprehensive assessments of patients to identify physical, emotional, and psychosocial needs.
• Develop, implement, and evaluate individualized care plans with patients, families, and team.
• Administer medications and treatments as prescribed, monitor for side effects and efficacy.
• Provide education to patients and families regarding disease, symptom management, and end-of-life options.
• Coordinate with physicians, social workers, and other healthcare professionals to ensure seamless care.
• Maintain accurate documentation of patient care activities in EMR.
