Responsibilities

• Assess, monitor, and document patient symptoms using required software.
• Collaborate with patients, families, and interdisciplinary team to develop individualized care plans.
• Advocate for patients, explain rights, procedures, and responsibilities.
• Observe and evaluate patient status, safety factors, and family dynamics to identify caregiver education needs.
• Supervise and direct aides and licensed practical nurses per guidelines.
• Participate in on-call rotations and provide back-up assistance as needed.
