Job summary

UPMC Family Hospice and Palliative Care in the State College area is hiring an RN to deliver direct one-on-one patient care in home-based settings. The role emphasizes life-changing medicine within a collaborative team including physicians, social workers and nursing staff.

Shifts run Monday through Friday, 8:00 am to 4:30 pm, with on-call, holiday, and weekend rotations. Final title and pay depend on education, experience, and certifications. Mileage reimbursement is provided.

Qualifications

• Current RN license required.
• Hospice/home health experience preferred.
• Ability to collaborate with physicians, social workers and other staff.

Responsibilities

• Coordinate case management with an interdisciplinary health care team to ensure appropriate patient services.
• Provide direct patient care and adjust the care plan with physician input.
• Complete documentation accurately and promptly in the electronic system during home visits.
• Participate in weekend and on-call coverage as required.
