Overview

Works under the supervision of the Nurse Manager and the Director, responsible for assessment, planning, implementation, and evaluation of total nursing care for assigned patients on an intermittent visiting basis. The RN may work in various settings within Riverside Health System, primarily in a home care environment. Utilizes communication, critical thinking, problem solving, and decision-making skills to create individualized care plans considering physiological, psychosocial, and cultural factors. The RN On-Call provides after-hours support via phone or patient visits, triages patient needs, and manages referrals, ensuring timely and appropriate care. Responsible for comprehensive assessment of patients' physical, functional, ADL, psychosocial, spiritual, communication, cultural, and environmental needs, either via phone or in person. Works effectively within RHCD to maintain a patient-focused environment of excellence, providing compassionate, quality care aligned with Hospice Philosophy.

What you will do

• Assess, plan, implement, and evaluate nursing care for assigned patients on an intermittent visiting basis. Provide after-hours support via phone or visits, manage referrals, and oversee caseloads.
• Handle calls from clients regarding issues or concerns, triage needs to determine if a visit is necessary or if issues can be addressed through education.
• Complete all required clinical documentation accurately and timely, including patient notes, verbal orders, certifications, and recertifications, adhering to RHS standards.
• Develop and revise care plans, provide specialized nursing services, counsel clients and families, coordinate services, inform physicians of changes, and plan for discharge when appropriate.
• Conduct comprehensive assessments of patients' physical, functional, psychosocial, spiritual, communication, cultural, and environmental needs, utilizing standards of practice and performing OASIS assessments as needed.
• Supervise RNs and LPNs, participate in training, and contribute to multidisciplinary team discussions on quality and outcomes.
• Delegate and prioritize tasks to ensure quality patient care, respond to emergencies, and recognize support needs, utilizing available systems effectively.

Qualifications

Education

• Registered Nurse (Required)
• Bachelor's Degree in Nursing (Preferred)

Experience

• Acute Care experience (Preferred)

Licenses and Certifications

• Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) (Required)
• CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 days (Required)
• Valid Driver's License and ability to drive personal vehicle for RHS business at least 25% of the time.
