Job summary

Housecall Providers is seeking a Registered Nurse - Hospice to deliver professional nursing services under the plan of care established by the interdisciplinary team. The role includes patient evaluation, care planning, and coordinating with hospitals, facilities, and community providers.

This 0.8 FTE night-shift position requires an Oregon RN license, at least two years in RN medical/surgical settings, CPR certification, and a valid driver's license.

Qualifications

• Current unencumbered RN license in Oregon.
• Minimum of two years RN medical/surgical experience in an acute care setting, and/or a combination of medical/surgical, home health, hospice experience or long-term care experience.
• 1 year of QA/QAPI experience may substitute for 1 year of RN experience.
• CPR certification at hire or within 6 months in position.
• Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.

Responsibilities

• Perform initial and ongoing nursing evaluations and contribute to the plan of care.
• Coordinate and monitor all care provided, communicating changes to the team.
• Use clinical assessment skills to provide care for patients, families, and caregivers.
• Participate in IDT meetings for plan of care development and evaluation.
• Develop and implement a plan of care for each patient and evaluate it.
• Assist with goals of care discussions and directives as needed.
• Provide dietary counseling and address patient/family needs.
• Collaborate with IDT during crisis management and referrals.
• Document accurately in electronic medical records per regulations.
