Job summary

UR Medicine Home Care seeks a Hospice Registered Nurse for Night On-Call to support patients and families in their homes after hours. You will provide symptom management, guidance, and emotional support, and you will be called to visit only when needed.

Schedule: 4 nights on, followed by 3 days off, from 4:30 PM to 8:00 AM. Responsibilities include delivering care per the plan of care, responding to patient calls, and coordinating with the Hospice Medical Director as needed.

Qualifications

• Licensed New York State Registered Nurse (RN).
• Minimum of one year experience as a clinical nurse in a hospital or home care setting.
• Hospice experience preferred.

Responsibilities

• Provide nursing care to patient/family/caregiver as directed and in compliance with all applicable laws and regulations.
• Receive patient related calls. Ensure problem resolution at point of contact to the maximum extent possible. Execute good decision making and set priorities.
• Provide home visits as needed for gathering patient information, treatment and support. Visits may include pronouncement of patient death and crisis management (in consultation with the Hospice Medical Director or Hospice Management as needed).
• Provide patient services based on plan of care together with identifying other physical, psychological, social and spiritual needs for assigned patients.

Are you a compassionate RN who finds purpose in supporting patients and families during life’s most critical moments? UR Medicine Home Care is seeking a dedicated Hospice RN for Night On-Call to provide expert, compassionate care when it's needed most.

In this vital role, you’ll be the calm and capable presence patients and families rely on after hours—offering guidance, symptom management, and emotional support in the comfort of their homes. This is an on-call position, meaning you will only respond and make a home visit if a patient need arises.

Schedule: 430PM to 8 AM, one week is 3 days and the following is 4 days.

The Hospice RN Night on Call will:
