Job summary

ACG Hospice in Louisiana is seeking a dedicated Registered Nurse to evaluate patients and develop individualized care plans, while communicating with patients, families, and the care team.

As a driving force for our care team, you will coordinate care to ensure quality hospice services, provide ongoing support, and help patients and families navigate illness with compassion. We offer substantial professional development, leadership support, and a comprehensive benefits package.

Qualifications

• Graduate of an accredited school of nursing with a current state license as a registered nurse.
• 2+ years of nursing experience in a clinical care setting (hospice experience preferred).
• Reliable transportation.
• Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.

Responsibilities

• Evaluate patients and create individualized care plans.
• Communicate with patients, families, and the care team.
• Coordinate with the care team to ensure quality hospice care.
• Provide ongoing support and adjustments as patient needs evolve.
